D = Data.
The header on the board is for two USB sockets; the list you've given is for
one and a bit.
On the pin outs, I've listed P5 and P6; these are the two USB ports, each
having a + and - input. So connect +D to P5+, and -D to P5-.
If you're lucky, the other 3 connectors may include another pair of +D
and -D. If so, connect the second set as +D to P6+ and -D to P6-
