What is the guideline to use to determine which of the two IDE controllers
on a MB is the primary and which is the secondary?

if you look carefully at the motherboard (you may need a good light and some
reading glasses)
the channels should be labeled...

if not...just put a drive on one channel
then watch the bios messages
and you will see which channel the device is detected on<!-- ~MESSAGE_AFTER~ -->

usually the board is marked IDE1 and IDE2

Uhhh, let me think. I believe on the motherboard itself it will specify ide1
and ide2. From that we can infer through consultation with a magic 8 ball
that the primary would be ide1. No ide2. No wait... Yep ide1. And by the
process of elimination that leaves us with ide2 being the, you guessed it,
secondary.
